Do No Harm: A Doctor Simulator with a Lovecraftian Twist

As the sole doctor in an isolated 19th-century village, your job is simple—diagnose patients, administer treatments, and keep the villagers – and yourself – alive. But as the days pass, reality itself begins to shift.  Entities begin warping reality, blocking key tools, deceiving you about treatments, and distorting your perception of what’s real.

That’s the premise of Do No Harm, an upcoming doctor simulator with a Lovecraftian twist, both in its aesthetic and its unsettling, choice-driven narrative. Developed by Darts Games and co-published with Hawthorn Games, this game draws heavy inspiration from Papers, Please and Death & Taxes, demanding careful decision-making under pressure.

To survive for 30 days, you must analyze symptoms with precision, consult your Book of Medicine, and make life-or-death choices based on incomplete information. As patients’ ailments grow stranger and out of the expertise of your books, you may even have to delve into ancient and cryptic medical procedures and hope that everything works out just fine.

Every interaction shapes your reputation, influencing how villagers act towards you—and ultimately leading to one of 12 possible endings.

With its mix of strategy, horror, and psychological tension, Do No Harm looks to offer a unique spin on the medical sim genre. The game launches on Steam on March 6th, but you can get a taste right now with its free demo
